Official Rules — $500 Amazon Gift Card Giveaway

This giveaway (the “Promotion”) is sponsored by [SPONSOR LEGAL NAME], [SPONSOR MAILING ADDRESS] (the “Sponsor”). The Promotion is in no way sponsored, endorsed, administered by, or associated with Amazon.com, Inc. or its affiliates, or with any government agency.

The Promotion is open to legal residents of the fifty (50) United States and the District of Columbia who are eighteen (18) years of age or older at the time of entry. Employees, officers, and directors of the Sponsor, and their immediate family members and household members, are not eligible.

Entry is void where prohibited or restricted by law. Entrants are responsible for complying with any laws applicable to them.

The Promotion runs as an ongoing monthly drawing. Each monthly entry period begins at 12:00:00 a.m. Eastern Time on the first calendar day of the month and ends at 11:59:59 p.m. Eastern Time on the last calendar day of that same month (each, an “Entry Period”).

One (1) prize will be awarded per Entry Period. Entries are specific to the Entry Period in which they are received and do not roll over. An entrant who wishes to be considered in a subsequent month must enter again during that month's Entry Period.

The Sponsor's computer is the official time-keeping device for the Promotion. The Sponsor reserves the right to modify or end the Promotion at any time, subject to Section 12.

Online entry: During an Entry Period, submit your email address (and, optionally, your first name) using any giveaway entry form on this website. Submitting the form constitutes one (1) entry and your agreement to these Official Rules.

Limit: One (1) entry per person per Entry Period, regardless of the number of email addresses used. Entries generated by script, macro, bot, or other automated means are void.

Free alternate method of entry: To enter without submitting an online form, hand-print your full name, mailing address, email address, date of birth, and the words “$500 Amazon Gift Card Giveaway” on a plain 3″ × 5″ card and mail it in an envelope with sufficient postage to [SPONSOR LEGAL NAME], [SPONSOR MAILING ADDRESS]. Mail-in entries must be postmarked within the Entry Period and received within seven (7) days of the end of that Entry Period. Each mail-in entry must be mailed separately. Mail-in entries receive the same chance of winning as online entries.

One (1) prize per Entry Period: a $500 Amazon.com Gift Card. Approximate Retail Value (“ARV”): $500 USD. Total ARV of all prizes awarded depends on the number of Entry Periods held.

The prize is awarded “as is” with no warranty or guarantee, express or implied, by the Sponsor. Gift cards are subject to the terms and conditions of their issuer. No cash equivalent, substitution, or transfer of the prize is permitted, except that the Sponsor reserves the right to substitute a prize of equal or greater value if the advertised prize becomes unavailable.

For each Entry Period, one (1) potential winner will be selected in a random drawing from among all eligible entries received during that Entry Period. The drawing will take place within seven (7) days after the end of the Entry Period. Odds of winning depend on the number of eligible entries received.

The potential winner will be notified by email at the address submitted with their entry, and must respond within seven (7) days of the date the notification is sent. If a potential winner does not respond within that period, is found ineligible, or the notification is returned as undeliverable, the prize may be forfeited and an alternate winner selected at the Sponsor's discretion.

The Sponsor may require a potential winner to sign an affidavit of eligibility and a liability and publicity release before the prize is awarded, where permitted by law.

All federal, state, and local taxes on the prize, and all other costs not expressly stated as included, are the sole responsibility of the winner. Where required, the Sponsor may report the value of the prize to tax authorities and may require the winner to complete an IRS Form W-9 or equivalent before the prize is released.

Information submitted with an entry is collected by the Sponsor and used in accordance with the Privacy Policy.

Entering this Promotion also subscribes you to marketing email from GOV Health Report. You may unsubscribe at any time using the link in any email we send, and unsubscribing will not affect an entry you have already submitted. We do not sell your personal information.

Except where prohibited by law, acceptance of a prize constitutes the winner's consent to the Sponsor's use of their first name, last initial, and state of residence for promotional purposes without further compensation.

By entering, entrants release the Sponsor and its affiliates, officers, directors, employees, and agents from any liability for any loss, harm, damage, cost, or expense arising from participation in the Promotion or acceptance, use, or misuse of the prize.

The Sponsor is not responsible for entries that are lost, late, misdirected, incomplete, or not received for any reason, nor for technical failures of any kind, including malfunctions of any network, hardware, or software.

If the Promotion cannot be conducted as planned for any reason — including infection by computer virus, tampering, unauthorized intervention, fraud, technical failure, or any cause beyond the Sponsor's reasonable control — the Sponsor reserves the right to modify, suspend, or terminate the Promotion. If the Promotion is terminated before an Entry Period's scheduled end, the Sponsor will, where practicable, award the prize for that Entry Period in a random drawing from among eligible entries received up to the time of termination.

All issues concerning the construction, validity, interpretation, and enforceability of these Official Rules are governed by the laws of the United States and the state in which the Sponsor is located, without regard to conflict-of-law principles. By entering, entrants agree that any dispute will be resolved individually, without resort to any form of class action.
